JOB DESCRIPTION

Assisted Living Charge Nurse

• Complete Functional Capacity Screen on all new residents upon day before or day of admission and every 365 days or upon significant change
• Complete Negotiated Service Agreement and review with AL operator, Family and Resident
• Complete Geriatric Depression Scale, Bed rail Assessment, Medication Self Administration Assessment, Mini Mental Assessment, Service Plan -to obtain the level of care necessary
• Review Med cart for State Compliance
• Review Refrigerator for food dates
• Review Resident rooms for Medications in rooms monthly
• Evaluate Residents for ability to self-administer medicati.ons upon sig change and every 365 days
• Coordinate with Assisted Living Operator when sig changes occur

MDS Assessment Coordinator

• Implement, Review and Adjust Care Plans
• Complete necessary Assessments in a timely manner for the MOS Coordinator and Collaborate with the MOS Coordinator
• Update Team Sheets appropriately
• Rotate on call with DON and work in collaboration with Director of Nursing
• Infection Control Certification with (NHSN Reporting)
