JOB DESCRIPTION

CLINICAL COORDINATOR

The clinical coordinator provides clinical leadership to the neighborhood team to ensure the clinical practices in the neighborhood meet regulatory compliance and nurture a person-centered culture while promoting individual and team learning in the neighborhood.

The clinical coordinator is responsible for the oversight and clinical care coordination of each resident with other nursing staff in the neighborhood. They provide knowledge and skills to clinical team members in specific areas of clinical accountability as needed and assure care is based on the personal goals and direction of individual residents.

The clinical coordinator works closely with the Neighborhood coordinator to pull the neighborhood team together to create home and coordinate day to day life in the neighborhood. They see their role as a role of service to other team members, taking initiative to assure resources are available to the team and to assist in navigating obstacles they may encounter while providing person-centered care.

Clinical oversight

  • Ensure the clinical operations in the neighborhood are in compliance with federal, state and local regulations, the Nurse Practice Act of the state and professional standards of nursing care while honoring person-centered care, including but not limited to: assessments, medications, treatments, physician orders
  • Provide leadership to clinical team members on specific areas of clinical accountability such as infection control, safety, wound management, falls, RAI process or other areas as needed.
  • Guide the implementation of clinical policies and procedures which ensure safe, sanitary and efficient practices supporting professional nursing care standards.
  • Assure resident care is directed by residents, centered on their preferences and routines and supports their choice and dignity.
  • Coordinates the care of each individual living in the neighborhood.
  • Assures documentation is in compliance with facility standards.
  • Completes incident reports as indicated and communicates with the director of nursing and administration as appropriate.
  • Maintains working relationships with physicians and other medical personnel in support of quality clinical care in the neighborhood.
  • Assists visiting physicians in making rounds in the neighborhood.
  •  Guides clinical team members to assure proper training, clinical competency, coaching and education are maintained in a manner supportive of team and professional standards of practice.

  • Coordinate team involvement in QA process

  • Provide leadership to team in audit corrections

Team coordination with Neighborhood coordinator:

  • Participates in interview and selection of new team members.
  • Assures direct caregivers are involved in the interview and selection of new team members.
  • Provides orientation and training to new team members.
  • Facilitates and documents coaching sessions as necessary.
  • Fosters collaboration consensus and negotiation to address problem solving in the neighborhood.
  • Facilitates conflict resolution as needed
  • Assists team members with personal development plans.
  • Oversight of self-scheduling process
  • Supports neighborhood team in assuring necessary schedule coverage.
  • Maintain staff attendance records
  • Coordinates team efforts in maintaining staffing levels.
  • Coordinate neighborhood leadership team
  • Facilitate regular team meetings and neighborhood learning circles.
  • Coordinates neighborhood staff TB’s.

Direct care

  • Actively provide direct nursing care as outlined on Neighborhood nurse job description.
  • Actively assists caregivers in providing direct care as requested/needed to residents.
